My Player was injured, what do I do?

How do I make an insurance claim?

Concussion/Head Injury: 

IC 20-34-7 states that an interscholastic student athlete, in grades 5-12, who is suspected of sustaining a concussion or head injury in a practice or game, shall be removed from play at the time of injury and may not return to play until the student athlete has received a written clearance from a licensed health care provider trained in the evaluation and management of concussions and head injuries, and at least twenty-four hours have passed since the injury occurred.  YPAC ADDED:  ANY player in ANY grade who is suspected of sustaining a concussion or head injury in a practice or game… 

Please take your player and the concussion evaluation release form to a licensed health care provider for clearance.  Your Coach or the Athletic Director will fill out the top part of the form before you see a doctor.

Suspected Cardiac Event:

IC 20-34-8 states that a student athlete who is suspected of experiencing symptoms of sudden cardiac arrest shall be removed from play and may not return to play until the coach has received verbal permission from a parent or legal guardian for the student athlete to return to play. Within twenty-four hours, this verbal permission must be replaced by a written statement from the parent or guardian.

Insurance claim for Player:

YPAC player insurance is “SECONDARY” to your personal insurance provider unless you don’t have personal insurance then it is the primary. The insurance ONLY COVERS THE PLAYER during official game & practice times. It does not cover siblings or other persons injured on the property at any time. Per the agreement on the registration form, neither YPAC nor the SCHS can be held liable. YOU are responsible for your player outside of the official game & practice times and any other guests and siblings. i.e If a player is injured at 5:55 because they are goofing off before the official practice starts at 6:00, YPAC insurance cannot be used.

To make a claim:  You must fill out an insurance claim form and submit it to our insurance provider.  You can get the forms from your head coach or any YPAC Staff member.  There is a page that must be completed by YPAC before you can submit the claim.  The forms are easy to fill out and contain all of the instructions necessary to file a claim as well as a checklist.  You can also print a copy of the form yourself.
